Eficácia da neuromodulação no tratamento da enxaqueca crônica: uma revisão sistemática

Introdução: As técnicas de neuromodulação no tratamento da enxaqueca crônica estão sendo bastante discutidas, visto que os tratamentos convencionais apresentam baixos índices de efetividade. Apesar de existirem inúmeras terapêuticas para a enxaqueca, sendo baseados em prevenção e sintomáticos, como anti-inflamatórios e triptanos, essas alternativas apresentam efeitos colaterais indesejados, são mal toleradas e não são responsivas. Nesse contexto, a neuromodulação está sendo relatada na literatura como eficaz para dores de cabeça. Objetivo: Avaliar a eficácia da neuromodulação no tratamento da enxaqueca crônica. Método: Trata-se de uma revisão sistemática da literatura, que teve como pergunta norteadora “Em pacientes com enxaqueca crônica, a neuromodulação em comparação com o placebo colabora para o alívio dos sintomas?”. Os artigos selecionados foram extraídos das seguintes bases de dados: Biblioteca Virtual em Saúde (BVS), National Library of Medicine (PUBMED), Scientific Electronic Library Online (SciELO), ScienceDirect e Cochrane Library. Como critérios de elegibilidade, foram escolhidos ensaios clínicos randomizados com idioma inglês, português e espanhol, dos últimos 10 anos e cm texto complete disponível, sendo a amostra final de 11 artigos. Resultados: Quanto aos resultados alcançados, observa-se redução significativa da dor na enxaqueca crônica com o uso do dispositivo de neuromodulção, como a estimulação nervosa periférica dos nervos occipitais, a qual revelou eficácia de 12 meses na redução dos sintomas. Ademais, a incidência de eventos adversos relacionados ao dispositivo foi baixa. Conclusão: A neuromodulação tem se mostrado como uma alternativa para o tratamento da enxaqueca crônica, provocando resultados indubitavelmente positivos entre os pacientes, em especial àqueles que não respondem de forma eficiente aos fármacos e tratamentos convencionais.
